Origins and trend of DBA programs in North America

I am interested in the origins and evolution of the DBA degree in the United States.
There doesn't seem to be a history to be found online regarding the inauguration of the degree - first recognized program, profile of increase of DBA programs in the US/North America, general health of the industry for DBA programs.
Any help appreciated,

Hi Peter,
thank you for this interesting question. I am not sure about it. In fact, I always thought that Harvard University was the first institution to offer a DBA program.
Now, first I don't know if this is true; and second, the Harvard DBA is not a typical DBA but rather a PhD. The fact, that this program grants a DBA after graduation merely has to do with internal policies.
I just sent an email to Harvard University asking to bring me in contact with someone who may know more about the history of DBA programs. Will keep you updated.
